develop an monitoring plan for Onondaga Community Hospital.   The plan must identify at least 5 separate audits for inappropriate access or use by users of the electronic health record.  For each audit, you must provide an example of a potential risk, the purpose of the specific audit and at least one example that would be considered inappropriate.

NOTE:  It may seem reasonable to include an indicator for inappropriate release of information from the record; however, that would be an internal HIM audit based on employee work.  This assignment asks you to reflect on who may or may not be accessing the electronic medical record appropriately, and ways that we could monitor usage to determine if it was appropriate.

To facilitate the development of the monitoring plan, it is recommended you do not use a narrative format,  Use of a word table as shown below will ensure completeness.   You may use Word or Excel to create the table.

Audit Indicator:Purpose Examples of Inappropriate Access Example:  Monitor staff access to Celebrity Record during and after episode of care

Ensure all staff who accessed identified record were involved in patient care;

Ensure appropriate access to PHI by staff involved in patient care

(This purpose may be used for multiple indicators)

Access by RN scheduled to work on different floor from the patient care area where individual was treated.

Access of BAC laboratory report by occupational therapist as this information is not necessary for completion of OT Evaluation.

This assignment allows you some creativity in the audit indicator and in the examples.  Please be sure the focus is on validating appropriate access to PHI and/or identifying inappropriate access to PHI.
